New Patients
All new patients are required to provide a credit card and $50 deposit upon scheduling. We ask that you give us sufficient notice if you need to reschedule or cancel your appointment – which is 48 hours. If your appointment is on a Monday, you must cancel or reschedule by Friday at 2pm. We carve out extra time in the schedule for your first appointment. Also, lot of preparation in terms of scheduling and paperwork has been invested into your initial visit before you come to our office. If you cancel or reschedule with less than 48 hours notice, please note that the $50 deposit is non-refundable. When you arrive for your visit, you can apply the $50 deposit towards the self pay rate, the deductible, your co-pays or products.
Cancellation Fees
Cancellations and rescheduling must be done 48 hours in advance of your first/initial visit. We require 24 hours notice for all follow ups. If you late cancel for your first/initial visit, your deposit will be retained and is non refundable. If you cancel less than 24 hours for your follow up visit, you will be charged a $50 late cancellation fee. Monday appointments need to be canceled by Friday at 2pm.
We appreciate your understanding and courtesy with regards to missed appointments. We set aside this time exclusively for you.
Insurance
We bill your insurance as a courtesy. For out-of-network policies, we are happy to provide you with a superbill to submit to your plan (except for United which we can bill directly). We submit for your in-network acupuncture visits in a timely manner. If your insurance company does not remit payment to us after 30 days, we kindly ask that you that the patient call their insurance plan to try to expedite payment. After 30 days, the patient is ultimately responsible for payment and will be charged for any visits which have not been reimbursed by insurance. Also, if the insurance reimbursement is inadequate, we reserve the right to request the balance from the patient.
Did you also know that your FSA or HSA can be used for acupuncture? Use your benefits card to pay for your acupuncture session. Or, ask us for an itemized receipt to submit to be reimbursed from your FSA or HSA plan.
We accept Checks, Cash, Visa and MasterCard.
